I agree to provide a copy of my dog’s C5 vaccination certificate:*

I will not bring my dog to day care or grooming in the event that my dog displays symptoms of being unwell, e.g. unusually tired, vomiting and/or coughing and etc.*

In the event that my dog is unwell and I am not contactable, I authorize for Mr. Teddy Bear staff to take my dog to their preferred vet for treatment and I will pay the full expenses associated with that treatment when I collect.*

I agree that in the event that my dog arrives at day care with fleas &/or ticks, my dog will be Flea bathed & treated accordingly and I will be charged for any expenses.*

I will notify Mr. Teddy Bear of my dog’s pre-existing health conditions.*

Mr. Teddy Bear and any associated company is not liable for any injury that may arise in my dog as a result of a preexisting condition*

I accept all reasonable risk of injury to my dog that may occur in normal doggy day care or grooming activities*

I understand and agree that Mr. Teddy Bear shall not be held responsible for any damage to my property caused by my doggy(s) during the period in which they are in its care*

I will notify Mr. Teddy Bear staff in the event that my dog has recently displayed aggressive behavior towards other dogs or people*

In the event my dog is aggressive towards other dogs or staff members, at daycare or during grooming, I will be asked to collect my dog immediately*

I understand that if my dog is aggressive to other dogs or staff members, Mr. Teddy Bear reserves the right to refuse service*

I understand that I am liable for any medical care expenses and damages that result from injuries caused by my dog(s)*

I understand that Mr. Teddy Bear Trading hours are Mon-Fri 7:00am to 6:00pm and Saturday 8:00am to 2:00pm*

I will pick up my doggy(s) by 6:00pm Mon –Fri and 2:00pm on Saturday sharp. A late fee will be applied for late pickup.*
